What is a Hospice?
Hospice is a philosophy of care with an empahsis on improving quality of life. Hospice provides a framework for continuous, multi-disciplinary care. Our hospice services focus on quality of life, emotional support for the entire, in-home management of medical needs, and the advanced management of pain and symptoms.
Who Qualifies for Hospice?
Hospice care is for patients who are terminally ill or have a life-threatening illness determined by a physician. These patients include, but are not limited to, those having diagnoses such as cancer, end stage lung disease, iver disease, kidney disease, heart disease, end stage dementia, neurological disease, HIV, AIDS.
Where is Hospice Care Provided?
Hospice Services are designed to provide support to patients, their families and loved ones wherever they call home. This can be a Long Term Care Facility, Assisted Living, Retirement Community or at the patients’ own home. The goal is to maximize the patient’s quality of life.

Our Hospice Team
Our caring team of professionals and volunteers provide outstanding hospice care. The hospice team follows a personalized plan of phycial, emotional, and spiritual care under the supervision of the patient’s physician and our hospice medical director. The patient’s plan of care is continously renewed and adapted to meet the patient’s changing needs.
The Team Includes:
Physicians: The hospice physician works in conjunction with the patient’s physician to provide pain and symptom management.
RN Case Managers: Coordinate care by making home visits to monitor the patient, teach caregivers and provide skilled care, comfort and support.
Social Workers: Help the family understand and cope with the related emotional financial stress and provide counseling.
Chaplains: On hand to offer spiritual support for patients and their families.
Volunteers: Provide a variety of care from companionship, support, errands, and offer temporary relief to the family.
Home Health Aides: Provide personal care.
Nursing On-Call Services: Available 24/7 to deal with crisis situations, answer questions and conduct visit when necessary.
Hospice In-patient Care: For short term symptom management at skilled nursing facilites.
Respite Care: For Temporary relief to caregivers.
Continuous Care: For brief periods of acute pain and symptom management at home settings.

Admissions to Moonlight Hospice Care, Inc. is based on the patient’s needs rather than the ability to pay.

The sooner we start, the more effectively we can help enhance the quality of life for our patients and their families. Our staff is available to work with the family and physician to determine when a patient can benefit the most from our hospice care.

Helping Loved Ones Through Grief
Grief can affect family and friends for a long time. Our concern for our patients’ loved ones continues during the time of bereavement. Our staff maintains close contact throughout the year to assist in any way possible.

10:00 AM to 5:00 PM
Monday through Friday
A 24 Hour - 7 Days a week on-call service is available to direct your calls to our professional staff.
Essential nursing, therapy and hospice aide care is also provided on Saturdays, Sundays and Holidays.
